    First Year-Chapter 9

    Chapter 9

Things started off pretty smoothly in the CCU as Henry predicted. Dr Wishnia was off the chain in a humorous way. Rounds with her were quick and to the point, there was no pimping here. I was able to relax, take care of my patients and spend some time getting to know them. Part of our responsibility this month was reading EKG’s. Each intern had to read at least 100 EKG’s. Janice planned to read at least 200 to show the level of seriousness that she took with her future career in Cardiology. Dana and I found Cardiology interesting but we certainly were not as driven to it as Janice. Janice knew every significant recent study that was published about acute myocardial infarctions or congestive heart failure management. I wasn’t up on every study but I did my fair share of reading so I wouldn’t look too bad on Wishnia rounds.

I actually found I liked Henry and often on down times, I had lunch with Dana, Tina and her “sometimes boyfriend”. Tina continued to flirt, or perhaps I interpreted it as flirting, with me. Maybe it was just her way of being friendly. She had moved on to be the ICU senior resident. This was her forte. She thrived on it. In the ICU she apparently was functioning like a fellow and the fellow over them was skating along pretty easily and was focusing on the easy work of bronchoscopies and interpreting pulmonary function tests. Among Tina’s residents was Bette. Although Tina was trying to be politically correct, she let on that Bette was not thriving in the pace and difficulty of the ICU. Now it was now her turn to get her buns chewed off by her ICU attending.

Apparently Bette, the glamour girl, had pissed off the nurses and they were not cooperating with her or trying to make her life easier by taking any extra steps to help her out in her work. She refused to listen to any advice from them and she had apparently taken an “I’m too good for this”, attitude. We were on call on the same every third night but while my nights were easy and allowed me to read and sleep. Her nights kept her up with the nurses suddenly becoming ignorant of what to do on each of her nights. They called her all night with every abnormal lab or elevated blood sugar. The more resistant they were, the angrier and bitchier she became. The brown Barbie became the scorn of the nurses and she hated them in return. Her reputation of being difficult to work with traveled through the hospital and by the middle of the second month of her internship, she’d become the glamorous scourge for the entire nursing staff of the hospital.

    1. I really love this story. For one, you have brought the beautiful Tina into the mix. I know nothing about the medical field except what I see on tv or observe from personal experience. I enjoy learning thru your eyes what interns must go thru to become doctors and throwing in our beautiful l-word ladies helps alot too. Thanks! PPS!!

    2. Ok Squirly maybe were not supposed to go here but I want to know what happened to JJ. Do interns find that kind of stuff out? How could you learn if you don’t know? Is Uncle Phil ever going to address his comment to Helena or do interns just have to let that stuff go and move on? Did Dylan do something she is a little ashamed of or did she find something in her google serch about Hels that upset her or did mummy do something? Hum Now onto a convergence with Carmen, months and months is a long time to go with out any fervent touch so maybe bygones should be just that. Hope to see you on the same bat channel at the same bat time tomorrow. Thank you, Md, akd Oma

    3. I think Helena’s feelings for Carmen are more then just sexual. Mummy Peabody done her job well….not only did she help Carmen into the Peabody clinic program she also proceeded to put a wedge in between Helena and Dylan. For some reason I don’t think Carmen getting the intern was a coincident. I still think Helena and Dylan should take a brake from one another until their lives get stable. I just don’t think they can handle their current situation at this time. I know there have been people who have been in their shoes and have come out ok but I don’t think a long distance relationship is for them(especially Helena). I once considered going into pediatric when I was in high school but when my mom told me how long I had to go to school I decided on the IT field. I think I mad the right decision after reading some of this story besides I don’t think I could take a child dying on me.

    4. Md-The short answer of is shit happens. People do arrest with cardiac cath’s. It could have been an irregular rhythm, he already had some the night before, could have torn the coronary artery, cold have had an allergic Rx, could have pushed a little plaque and trigger an acute MI. JJ was a real patient and I don’t know what happened. Neither will Helena. Shit happens. As far as Dylan’s issue we won’t know for a while. ;-)

    5. RB-Very interesting theory you have about MamaP and Carmen. Several of the issues you raise, including the death of a child will be addressed later. Sorry for so much death. But what do they say. There are only 2 things we can’t avoid, death and taxes.

    6. Absolutely awesome! Umm when is Bette gonna get the memo that the medical staff is an extension of her hands, ears and eyes? Is a good ole fashioned spanking on the way? Oh right, Kennard is tearing her a new one…wait you said chewing her buns! Lol!Thanks for sharing this Squirl! I really respect and appreciate what MDs do…what any service professional does, what they go through and what they have to see. It would be interesting to hear her uncle speak to his comment, but I suspect that the jist is that in order to function, you do have to accept certain truths about life and death. I think in the way that we as a society have become more and more acclimated to catastrophe and violence, medical professions too become acclimatized to their work environment. Even with that, I don’t suspect that there is anyway that it doesn’t impact them on a deeper level…add to that depending on work environment, they have ridiculous schedules and at times some have touch and go family lives…. They have to find means of coping less the stress, craziness, and moments of impotence comes out in other ways. I’m thinking non-hospitalists may possibly have an easier go of it? after residency? Dang, they won’t see each other for months now…wow! Come on you can introduce some sci-fi and bring in a 3D hologram for these suffering ladies!! Lol! PPS

    7. Squirl, what has gotten into you? OMG!!!First you make my beloved Bette straight (just kill me now — and may I remind you that she secretly has feelings for the Hot Helena and is really gay) and now she has alienated herself against the entire hospital nursing staff??? WTF?? Even people that have never worked in a hospital setting know this is the kiss of death, particularly if you are a resident/intern. I just have to let you know that I am weeping at the thought of what you might do next (God forbid the woman should get married, pregnant, or leave residency). Dylan is acting strangely distant. I’m beginning to wonder if she isn’t having a friend with benefits on the side as well. Hmmm… I think the way you are incorporating actual medical practice into the story is really intriguing. Oh, and definitely sign me up for a Carmen look-alike playmate! :>)

    8. Okay Squirl, enjoyed the update, but is this gonna be a BeLena or what? Not to be pushy or anything, caus’ I wouldn’t (smile) but let me know somethin’ !!!

    9. Dont know much about the medical field. My husband had a Heart Transplant in 92 and passd away in late 07,but it wasnt his heart. So that the entend of my knowledge. see by the way you write that becoming a Dr is not an easy career field to go into. My girl Dylan, might have a pillow friend, I hope not. Helena cant say much but it will still hurt both if its true. Bette better get off her Hi=horse an get with the nurses, they are the back bone ,eyes, hands of a Dr. thanks

    10. yes *big* sis… drop me a line telling me about your progress I’ve been worried sick about you… well you know more so then I have been myself… so yeah drop me a line… cause well I need to know… know what I mean your a special part of my life I hope you realize that… huggles, JC

    11. Hello My Most Passionate Friend… Why oh why must you make me think with my mind splitting and my lungs filling… why oh why oh thats right cause you are so ‘fucking’ amazing… trust you will get a full detailed *cough* *cough* JC overview within the next day or two… you are just out of this world fucking amazing… and I’ll say it again… ‘FUCKING’ amazing… my mind is in circles with your words… if the last one didn’t prove it… this one better you are a writer 100% pure and sure… I’ll put my life on it… love and hugs, JC

    12. S what a great chapter about how Dylan saw how the Drs and nurses saw that youn woman like if nothing happened. But after awhile like any other job it’s just come and go and nothing more, sadly. I am thinking that Dylan also had a one night stand as Helena or even maybe something more? As for Bette, dang that is what happens when you think you know everything and need help no matter what, we ALL need HELP! great chapter S,

    13. Is living in Cleveland getting to Dylan? Yeah, upsetting things happen in hospitals everyday, people die in hospitals every day. It’s a tough balance to care enough to do a good job and to be able to distance yourself enough to do a good job.

    14. Great update! You know these two really need to talk. Face to face, because they are in trouble and they know they are. And by the way are you going to make Bette a little humble. It will help on her character I think. You know take that ego down a notch or two.
